To expedite the check in process, please print out and complete these downloadable forms before your first appointment. If you are unable to do so, please call our office and we will mail the forms to you.

Insurance

We will be happy to assist in helping you obtain the maximum dental benefits you are entitled to and will file most dental insurance claims for you. Your insurance company will be billed direct by us, with payment due at the time of service for any amounts estimated to not be covered by your policy.

Payment

Payment is expected in full at the time of service unless other arrangements are made in advance. For your convenience we offer a variety of payment options including cash, personal check, MasterCard, Visa, Discover and American Express.

Interest Free Financing

You may also apply for the Care Credit Medical/Dental Card. Care Credit offers 3, 6 and 12 month interest-free payment plans, and extended financing up to 48 months. You can apply in our office or online.
